Initial History & Physical Exam
Our services support you and/or your loved one with comprehensive care after major surgery, hospitalization, skilled nursing facility admission or emergency room visit in the comfort of your own home.
Comprehensive transitional care begins first with in-depth, post-discharge home visits where a Nurse Practitioner works with you and your family to customize a health plan specific to your recovery and long-term health goals. We collaborate proactively with your surgeon, specialist and/or primary care provider, your family and/or residence facility staff to ensure that you receive quality care and resources requisite for your optimal health outcome.

We are a professional fee-for-service healthcare practice.
Our fees may be covered under your insurance plan. We recommend calling your insurance provider and inquiring directly.
